WELCOME TO OREGON NEUROLOGY
At Oregon Neurology, we take great pride in providing excellent care in a collaborative way. We value open and honest communication, take the time to properly diagnose your or your family member’s condition, and provide all the information you need to feel informed and involved in making decisions. It’s important to know that you, as a patient, have a choice when choosing a provider for your neurological care. At Oregon Neurology, it’s important to us that you feel welcomed, cared for and supported.
We ask that you be on time for your appointment. If you arrive after your check-in time, your appointment may need to be rescheduled. If you are unable to keep your scheduled appointment, we ask you to notify us 24 hours in advance; failure to do so may result in a charge. To schedule an appointment, please contact us at 541-868-9430 during our normal office hours: Monday through Friday, 8:30 am to 12 pm & 1:30 pm to 5 pm.
If you have an emergency, please dial 911 or seek immediate assistance at an urgent care facility or emergency room. For non-emergent matters that cannot wait for our regular office hours, please call your primary care provider.
PHONE CALL POLICY
Our office receives many patient-related calls daily. While we try to handle all urgent matters on the day the call is received, non-urgent questions may be deferred until your appointment when the provider will be able to review your concerns in detail. Questions not directly related to your neurological problems should be directed to your primary care provider. Telephone calls will be reviewed by a medical assistant and will be forwarded to your primary care provider or the neurologist on call. Calls after 3:30 pm will not be returned until the next business day, at the earliest. When returning your call, we will leave a detailed message on the phone number you provide upon your request. If you require a prescription refill, please contact your pharmacy and allow 48 business hours for our office to process the request. Refill requests made on Fridays will not be completed until the following Monday, at the earliest. We do not refill prescriptions outside office hours.
All test results will be reviewed by your provider and addressed at your return appointment, unless the provider feels there is a need to contact you sooner.

FEES AND INSURANCE
To maintain reasonable rates while providing state-of-the-art care, we have established the following policies regarding insurance and payment. Please call our business office at 541-868-9430, option 7, to make special payment arrangements, if necessary.
Please bring insurance cards to your appointment. We will bill your primary and secondary insurance if the insurance cards are presented at the time of your visit; otherwise you will be responsible for the charges until complete insurance information is provided. While we will help in any way possible, your insurance is a contract between you and your insurer. Please contact your insurance provider to verify your benefit level before each visit.
If your insurance plan requires a referral, please ensure this is in place by contacting your primary care provider 48 hours before your appointment with us.
You will receive a statement of your account once your insurance provider has responded. If you don’t have insurance, we require payment at the time of service unless special arrangements are made. We are a Medicare-participating provider and accept assignment for all of our Medicare patients. We charge a fee for completing disability insurance forms, depending on the complexity and frequency. This fee is not paid by insurance. Please contact medical records for details.
Patient Referrals: We can think of no greater compliment than being asked to care for your family or friends. Thanks for your referrals.
Provider Referrals: If you have been referred by another provider, that person will promptly receive a letter outlining our findings and the course of treatment. We encourage you to return to your primary provider after our specialized care is complete.
