OFFICE VISITS
Please call (541) 868-9430 during normal office hours to make an appointment. Our hours are Monday through Friday, 8:30 a.m. to noon and 1:30 p.m. to 5 p.m.
If you are unable to keep your scheduled appointment we ask you to notify us 24 hours in advance. Failure to do so may result in a charge.
EMERGENCIES
If you have a medical emergency after hours, please call (541) 868-9430.
Our service will contact the doctor on call who will return your call promptly. If you need immediate attention, please go directly to the nearest hospital and ask that your physician be contacted.
MEDICATION
Please plan ahead. Keep your prescription numbers and call your pharmacy 48 hours in advance when your medication requires refilling. Our doctor on call can refill only essential medication after hours and on weekends. Narcotic precriptions will only be filled during office hours.
FEES AND INSURANCE
To maintain reasonable rates while providing state-of-the-art care, we have established the following policies regarding insurance and payment. Please call our business office at (541) 868-9433 to make special payment arrangements if necessary.
Please bring insurance cards to your appointment. We will bill your primary and secondary insurance if the insurance cards are presented at the time of your visit; otherwise you will be responsible for the charges until complete insurance information is provided.
While we will help in any way possible, your insurance is a contract between you and your insurance provider. Please contact your insurance provider to verify your benefit level before each visit.
If your insurance plan requires a referral, please ensure this is in place by contacting your primary care physician 48 hours before your appointment with us.
You will receive a statement of your account once your insurance provider has responded. If you don’t have insurance, we require payment at the time of service unless special arrangements are made. We are a Medicare participating provider and accept assignment for all of our Medicare patients. We charge a fee for completing disability insurance forms depending on the complexity and frequency. This fee is not paid by insurance. Please contact the business office for details.
REFERRALS
Patient Referrals. We can think of no greater compliment than being asked to care for your family or friends. Thanks for your referrals.
Physician Referrals. If you have been referred by another doctor, that doctor will promptly receive a letter outlining our findings and the course of treatment. We encourage you to return to your primary doctor after our specialized care is complete.
